Indications: why take it?

  1. Skin infections due to staphylococcus and streptococcus:
  2. impetigo and impetiginized dermatoses:
  3. o as a local treatment only in localized forms with a small number of lesions;
  4. O in local adjunctive treatment combined with general antibiotic therapy adapted in the most extensive forms.
  5. Official recommendations for the appropriate use of antibacterials should be considered.

Contraindications: why not take it?

  1. This medicine MUST NEVER BE USED in the following cases:
  2.          – allergy to Mupirocin;
  3. · allergy to polyethylene glycol;
  4. · in application on the breast during breast infections, in case of breastfeeding.

Dosage and method of administration

  1. Dermal.
  2. Adults and children
  3. Dosage
  4. 2 to 3 applications per day after cleaning the infected surface.
  5. Duration of treatment
  6. The treatment will be continued for 5 to 10 days as an indication and according to the nature, the severity of the lesion, and effectiveness.
  7. Administration mode
  8. The treated area can be covered with sterile gauze or other occlusive dressing.
  9. Ointment.

Warnings and precautions for use

  1. Special warnings
  2. This ointment is not intended for ophthalmic or intranasal use.
  3. Risk of selection of Gram-negative bacilli (Pseudomonas).
  4. Precautions for use
  5. · The ointment is odorless, washes off with water, and does not stain.
  6. · When applying to the face, avoid contact with the eyes.
  7. Due to the presence of polyethylene glycol, which is absorbed in the event of an open wound and then eliminated via the kidneys, mupirocin ointment should be used with caution in patients with large wounds or burns and suffering from impaired severe or moderate kidney disease.
  8. · Mupirocin should not be used on extensive dermatoses (more than 20 percent of body surface area) or long-term in chronically infected dermatoses due to the potential risk of emergence of strains resistant to Mupirocin.

Pregnancy and breastfeeding

  1. Pregnancy
  2. Mupirocin can be considered if necessary during pregnancy, whatever the term. Indeed, although the clinical data are still insufficient to date, the animal data are reassuring.
  3. Feeding with milk
  4. This medicine can be used during breastfeeding.
  5. Avoid application on the breasts.

Interactions with other medicinal products and other forms of interaction

  1. Do not incorporate other active substances into this ointment, nor mix it with other dermal ointments, at the risk of dilution of the product leading to a reduction in the antibacterial activity and alteration of the characteristics of penetration through the skin from this mupirocin ointment, the other product or both, and loss of stability of the ointment.

Side effects

  1. A few rare cases of stinging and burning have been observed, always localized at the application site.
  2. Hypersensitivity reactions due to Mupirocin or its excipients, such as rash, urticaria, or angioedema, have been reported in rare cases.
  3. Reporting of suspected adverse reactions
  4. The reporting of suspected adverse reactions after authorization of the medicinal product is important. It allows continued monitoring of the benefit/risk ratio of the medicinal product. Health professionals report any suspected adverse effects via the national reporting system: the National Agency for the Safety of Medicines and Health Products (ANSM) and the network of Regional Pharmacovigilance Centers. 

Pharmacological properties

  1. Pharmacotherapeutic group: OTHER ANTIBIOTICS FOR TOPICAL USE, ATC code: D06AX09.
  2. Mupirocin is a naturally occurring antibiotic produced by fermentation of Pseudomonas fluorescens.
  3. It has an original structure unlike any of the antibacterial agents currently available in the clinic.
  4. It inhibits bacterial protein synthesis by reversible and specific binding with isoleucyl t-RNA synthetase.
  5. Due to this original mode of action, there is no cross-resistance with other families of antibiotics. In addition, in vitro tests have shown a very slow evolution of the emergence rate of resistant strains.
  6. Mupirocin at concentrations achievable in topical use is bactericidal in species sensitive to this antibiotic.
  7. SPECTRUM OF ANTIBACTERIAL ACTIVITY
  8. The critical concentration separates sensitive strains from resistant strains:
  9. S ≤ 2 mg/l and R > 2 mg/l

  1. The prevalence of acquired resistance may vary geographically and over time for some species. Therefore, it is useful to have information on the prevalence of local resistance. These data can only orient the probabilities of the sensitivity of a bacterial strain to this antibiotic.
  2. When the variability of the prevalence of resistance in France is known for a bacterial species, it is indicated in the table below:

  5. SENSITIVE SPECIES
  6. Gram-positive aerobes
  7. Staphylococcus aureus
  8. Beta-hemolytic streptococci
  9. The emergence of mupirocin-R strains is possible.
  10. The potential risk of a nasopharyngeal ecological impact with the selection of flora, especially vis-à-vis Gram-negative bacilli, including Pseudomonas, has not been assessed.
  11. After oral administration in man, Mupirocin is very poorly absorbed.
  12. After oral or parenteral administration, Mupirocin is rapidly metabolized to metabolites without antimicrobial activity. Consequently, it is reserved exclusively for local use.
  13. After applying carbon-14 labeled ointment to the skin of a healthy subject followed by 24-hour occlusion, there is no measurable systemic absorption of the antibiotic; antimicrobial activity persists in the outer layers of the skin up to 48 hours after application.
  14. The inactive metabolite: monic acid produced by skin contact is detectable up to 1 week after application.

Duration and special precautions for storage

  5. Do not keep any tube open for more than 15 days.
  6. Do not incorporate other active substances into this ointment or mix it with other dermal ointments.
